Current school forms are available for you below. Please download any form you require and either e-mail it to the school or take a hard copy into the school office.

Should your child be attending school and need to take medicine, we must remind all parents/guardians that we are only permitted to administer medicine which has been prescribed by a doctor.

When ordering any school uniform, please make sure that the relevant money is attached to the form. All cheques to be made payable to "Olney Infant Academy".
